I _ hereby give Yolanda Campbell Lifter the authority to act in my behalf to acquire the necessary records to complete my genealogical research project. She has my authorization to obtain from archives, libraries, institutions, and any additional repositories all records, certificates, or documents relating to my ancestry, specifically including (but not limited to) certificates of birth, death, and marriage.

I hereby agree to pay her for genealogical research to be performed on my behalf at the rate of $40 per hour plus expenses (vital records, copies, postage, microfilm rental, mileage, etc.).

I am enclosing $200 for the first five hours of research. The next five hours of research and the expenses will be billed before the report is mailed. Upon completion of the initial ten hours, I will decide whether to purchase additional research hours.

I am enclosing any information I have already obtained (vital records, census records, family groups sheets, etc.) as well as a list of sources checked so there will be no duplication of research.

I understand that she may not be able to solve my genealogical problem(s) since the availability of information varies from county to county as well as from individual to individual.
